Home Care Options - Home & Health Care Services

Home Care Options offers assistance to individuals who are elderly, ill or physically challenged and may need additional medical support. A comprehensive network of in-home health care services is provided to uphold an individual’s right to remain in their own home for as long as desired. Choice in hiring the supports that are needed fully rests with the customer thus granting control to the individual regarding the manner in which services are provided. All services provided through Home Care Options are reviewed fully by our licensed Registered Nurse. Funding for these services come primarily from Medicaid (Title 19), Title 20, Medicare, private insurance, private pay, and others and is administered through the Department of Health and Senior Services, Division of Senior and Disability Services.

• Consumer-directed Services

Eligibility:

• These home and community based services are provided to eligible persons 60 years of age or older and to adults with disabilities between the ages of 18 and 59.
• Individuals must be eligible for funding through the Division of Senior and Disability Services.
