Realities Program - Partial Supports
Realities provides support to persons living independently in their community, needing 2-12 hours of individualized support per week. Support and instruction in activities of daily living encourages personal growth and independence in the home environment. Individuals who seek greater exposure to community activities, socialization and interpersonal communication skills will benefit from the customized person-centered planning process offered through the Realities program. The program is funded by the Developmental Disabilities Resource Board in St. Charles County (DDRB).
Eligibility criteria for the program include:
Having a developmental disability
Residing in your own home, not with family members
Being a current resident of St. Charles County
Needing 2 - 12 hours of support per week
Being eligible for services from DMH- St. Louis Regional Center
Individuals in the Realities program can expect to receive assistance in the following areas as needed:
Cooking
Menu planning
Shopping
Community accessing
Roommate searches
House searches
Accessing funds
Accessing case management
Transportation
Health management
Advocacy
Bill paying
Budgeting
Problem solving
and More!
The specific services provided to an individual are identified in the person-centered plan (PCP). Each person will have the opportunity to determine his or her individual goals according to his or her needs and dreams and a responsibility to actively work toward those identified goals. There is a strong emphasis on the individual learning to access, develop, utilize, and maintain natural community and familial supports.
